In addition to the questions above, applicant must also supply the following to the ACCED-I office at ACCED-I, 419 Canyon Ave., Suite 311, Fort Collins, CO 80521 or Lori@acced-i.org. You will have the opportunity to submit the documents online after you click the Submit button.

- Three (3) Reference Letters to be completed and signed by clients

- A signed statement from the person who oversees your department

- Additional documentation including:
• Submit a sample of your operation’s response to a RFP that demonstrates all service offerings listed in the table below will be provided/coordinated by your department
• Submit a sample contract(s) that includes the following service offerings: meeting space, dining service/meal plan, catering, audio visual, and lodging
• Submit a sample marketing brochure or documentation (i.e. website) that demonstrates adherence to one-stop principles
